The EFSA NDA Panel concluded that a cause and effect relationship had been established between consumption of 10 mg/day of monacolin K from red yeast rice and the claimed cholesterol-lowering effect. That is a stronger regulatory statement than almost any supplement receives.

The catch is the dose. 10 mg/day is above the level the European Commission now permits in supplements, and it is the level at which EFSA identified significant safety concern.

EFSA also concluded that the composition and content of the minor monacolins in these products is variable, and that available data do not allow their contribution to the cholesterol-lowering effect to be evaluated.

Evidence against·Chemically identical to lovastatin

Monacolin K in lactone form is identical to lovastatin — the active ingredient of prescription medicines authorised across the EU for high cholesterol. EFSA states this plainly.

Taking red yeast rice can therefore expose you to monacolin K within the therapeutic dose range of lovastatin, 10 to 80 mg/day, without a prescription, without dose control, without liver monitoring and without anyone checking what else you take.

The French agency ANSES put it directly: because red yeast rice contains monacolin K, which shares the adverse effects of statins, plus other monacolins whose safety has not been established, consuming it exposes some consumers to a health risk.

EFSA judged the human adverse effect reports sufficient to conclude significant safety concern at 10 mg/day, and noted individual cases of severe adverse reactions at intakes as low as 3 mg/day. Reported effects include rhabdomyolysis and liver injury.

A fair counterweight: an analysis of the FDA and CFSAN adverse event systems from 2013 to 2023 found that of 363,879 musculoskeletal cases reported, those linked to red yeast rice accounted for 0.008%. The pharmacology is concerning; the observed reporting rate is low.

Both things are true, and the resolution is the same either way: this is a statin, so it deserves statin-level caution about interactions and monitoring.

Emerging·11% of a tested sample gave cause for concern

Thirty-five red yeast rice supplements bought from an EU e-commerce platform and registered online pharmacies were tested by validated LC-MS/MS. At least four (roughly 11%) gave cause for concern — three exceeded the European limits on total monacolin content, and there was severe bacterial contamination.

Products were also screened for deliberate adulteration with pharmaceutical lovastatin, by examining the ratio of monacolin K forms.

So most products were compliant. But when the compound in question is a statin at an uncontrolled dose, a one-in-nine failure rate is a different kind of problem from a one-in-nine failure rate on vitamin C.

Lovastatin equivalence
Monacolin K in lactone form is chemically identical to lovastatin. Therapeutic lovastatin doses run 10–80 mg/day.
Citrinin
A nephrotoxic mycotoxin produced by some Monascus strains. EU maximum in RYR supplements was cut from 2,000 to 100 µg/kg in 2020.
Do not combine
With other lipid-lowering therapy, or with drugs that raise statin exposure. The lovastatin product information warns of myopathy and rhabdomyolysis risk on interaction.
